1. Please download the card scanning registration form by clicking here.

2. Supply us with a completed FD-258 fingerprint card or click this link to purchase from our store

3. Please email a passport head shot photo if your agency is ACHA, FDOH, or DJJ. Email: info@pbnotarypublic.com

4. Physically mail the form with the completed fingerprints to us at – 1645 Palm Beach Lakes Blvd, Suite 1200, West Palm Beach, FL 33401.

5. Please provide us a mailing tracking number for us to track your mail properly. Email: info@pbnotarypublic.com

Once your card(s) are delivered we will review and process them in which the digital file conversion to Live Scan will occur according to the form and instructions and the fee will be determined by applicant location. Applicants will also receive a tracking number to follow up on the background check conversion via email.
