STEP BY STEP PHYSICAL MAILING INSTRUCTIONS
1. Please download and mail the card scanning registration form by clicking here.
2. Supply us with a completed FD-258 fingerprint card or click this link to purchase from our store

3. Please email a passport head shot photo if your agency is AGENCY FOR HEALTHCARE ADMINISTRATION, FLORIDA DEPARTMENT OF HEALTH, or DEPARTMENT OF CHILDREN & FAMILIES. Email: info@pbnotarypublic.com
4. Physically mail the registration form with the completed fingerprints to us at – 1645 Palm Beach Lakes Blvd, Suite 1200, West Palm Beach, FL 33401.
5. Please provide us a mailing tracking number for us to track your mail properly. Email: info@pbnotarypublic.com
Once your card(s) are delivered we will review and process them in which the hard card to live scan conversion t will occur according to the form and instructions. The fee will be determined by applicant location and an invoice sent before transmitting file to law enforcement per the background check. Applicants will also receive a tracking number to follow up on the background check conversion via email.
